This version is modeled on the one used in Brazil's 1996 Demographic and Health Survey. The main difference is that the categories for the outcome of the interviews come from the document that describes the AAPOR's Standard Definitions of quantities like response rate, contact rate, and refusal rate. Including these categories should allow us to compute the measures of survey quality that document describes.
Note that parts of the contact sheet may need adapting, depending on the management structure being used and so on. For example, I put in a field for an ID number, since I assume each interview will have some kind of identifier, but I don't think we know right now exactly how that will work.
